Learning Goal: I'm working on a health & medical discussion question and need an explanation and answer to help me learn.Create a scenario of an injured athlete with a brief history, observations, what could be found in the palpation, current status of range of motion, strength and possible diagnosis. This should be in SOAP note form. You are making up an injury and documentations. Please use the correct anatomy of goal writing. The SOAP note is the industry standard and how all medical notes are written. It literally needs to be written in the the below format with sections. It is important to remember that anything we do or perform during initial treatment and evaluation of the injury needs to be listed under the O section (this would include stopping the bleeding, splinting, CPR, etc). Anything we want the athlete to do or have done after initial treatment and evaluation of the injury should be listed in the P section (this may include x-rays, MRI, follow up with MD, etc). You may consider the following link for an example on how to write a correct SOAP note making sure to put the correct information in the correct sections: https://www.ncbi.nlm.nih.gov/books/NBK482263/ S: Subjective- anything the patient or bystanders tell usO: Objective- anything we do or perform during initial evaluation including immediate treatmentA: Assessment- our findings based upon information gathered through the S and O sectionsP: Plan- our plan for treatment of this injury after initial evaluation and immediate treatmentFor exampleS: Jenny is a 17 year old athlete who has suffered a grade 2 R lateral ankle sprain. She is a swimmer who twisted her ankle going down the stairs. Pt. states her pain is currently a 3/10 at rest and 5/10 in weight bearing. She has crutches but does not use them consistently.O: There is slight discoloration on the lateral malleolus with edema. ROM= Strength=. Ice and compression to R ankle x 20 min. ABC's x 2, ankle pumps x100, weight shifts side to side 2x25. Gait training with crutches on both flat surfaces and stairs.A: Pt presents with s/s of a R lateral ankle sprain.P: Cont with RICE at home, MD for x-rays to rule out fracture, to attend physical therapy 3 x per week x 4 wks.
