Complete a focused assessment and care plan on the two need areas that were not addressed in your first focused assessment (may be on a different patient). For example, if your first focused care plan addressed physiological & psychological needs, this focused care plan needs to address a sociocultural & spiritual need. Openly indicate which two of the four need areas you are addressing.
