10 sources Create a made up client to write an 8-10 page paper that will integrate the use of assessment, diagnosis, and use of an evidence-based practice model to create a treatment plan. The student will select a client from their field placement (if possible) as a subject for the paper. If the student is in a macro-setting in field, please speak with the instructor about creating a case student to utilize for paper. Parts of the Paper: Biopsychosocial assessment for the client with particular attention to presenting concerns. Include any symptoms and proposed DSM-5 diagnosis. Discuss the client’s presenting concerns as understood through the lens of a theoretical model (e.g. object-relations theory, attachment theory, cognitive-behavioral theory, etc.) Identify an evidence-supported treatment to be used with the client. This section should include a discussion around selection of the most appropriate therapy, taking into consideration: social work perspectives, values of the client, research indications for this client/client population, considerations for use with diverse populations Create an intervention plan based on the theoretical model and selected evidence-supported treatment. This section should include measurable goals, methods for reaching those goals, and a discussion of how success of the intervention will be evaluated (qualitative or quantitative).
